Ben Casey (medical drama, starring Vince Edwards)

     (ABC Primetime, 1961 - 1966;
      Syndicated)

Opening Billboard 1: "Ben Casey Motto Signature"

    [Most likely, this refers to the pretentious motto, stated by
     the announcer at the beginning of every episode, which was...
    "Man, Woman, Birth, Death, Infinity";

     Dan Matheson writes that the "quiet celestial" cue behind the
     opening Billboard (which is either this or the one below credited
     to David Raksin) was also recycled in the Capitol "Hi-Q" library
     under the title "4-TC-19";

     Since Gerald Fried was also a cue composer of music for the
     series "Star Trek" perhaps this was a foreshadowing of other
     "celestial" cues to come...:-)]

Opening Billboard 2: "Motto"

    [ASCAP title variations...
     aka: "Motto Theme";
     aka: "Motto Theme & Sig"...

     Most likely, this refers to the pretentious motto, stated by
     the announcer at the beginning of every episode, which was...
    "Man, Woman, Birth, Death, Infinity";

     It's possible that credit for this cue belongs to Gerald Fried;
     but since Raksin wrote the Main Title confusion at the publisher
     caused his name to get associated with it on ASCAP Cue Sheets,
     or there should be a shared credit with both composers;

     Dan Matheson writes that the "quiet celestial" cue behind the
     opening Billboard (which is either this or the one above credited
     to Gerald Fried) was also recycled in the Capitol "Hi-Q" library
     under the title "4-TC-19"]
